All Cub Scouts and Volunteers must complete the BSA Medical Forms Parts A&B. All Cub Scouts and Boy Scout Volunteers are required to submit the Shooting Sports Waiver. (See links below.) Registration is not complete until the additional the forms are completed and submitted.
